MyChart provides free and secure access to your health information 24/7. MyChart users can edit and manage appointments, complete patient forms and check in before exams, view reports, images, and medical records, and connect with other MyChart providers. TGH Imaging patients can access MyChart to view records of exams performed after August 1, 2023. Please note reports are posted after they are sent to your provider allowing time for review.

TGH Imaging offers convenient ways to access your medical record.
Your medical records include a written report which lists the findings and diagnosis of your imaging exam and the actual images from the exam, which are often requested by a referring physician and can be made available on a CD.

Request Copies of Your Medical Record
Patients may request their medical records directly via MyChart. If you do not have a MyChart account, click here to request an activation code and get started. You may also obtain a copy of your medical record by printing and completing the Request for Access to Protected Health Information by Individual Patients form below. The completed request form should be faxed or mailed using the information provided on the form. In most cases there is a charge for copies of your medical record. You will be informed of the charge before copies are provided.
There is no charge if records are released directly to your MyChart account.
There is no charge if records are faxed or mailed directly to a verified physician’s office.
The average turnaround time for a request is ten (10) business days. If you have questions about the disclosure of health information, please contact the Medical Records Department at (813) 847-3177.

Request Copies of Another Individual’s Medical Record
Third parties must complete and submit the Authorization to Disclose Health Information form to the TGH Health Information Management department. If you have questions about the disclosure of health information, please contact the Medical Records Department at (813) 874-3177.
